WHO WE ARE

Community
Hospices of America

Community Hospices of America operates
community based hospices in over 20 communities in nine states. Our
hospices are deeply rooted in their communities and have served these
communities for as long as twenty-six years. Our primary goals are to
provide the highest quality hospice care and service in every community
in which we operate and to be the hospice employer of choice in every
market in which we operate.

The Hospice Team will
assist the patient in arranging for medications, equipment, medical
supplies, acute inpatient hospitalizations, respite care, continuous
care, dietary counseling, homemaker services, and other services as
needed for the care and comfort of the patient and support of the
family. Community Hospices of America can also provide assistance in
seeking aid from other community resources.
